JOB DESCRIPTION: Facilitate patient and family groups. Provide support to family and patients as needed. Distribute program evaluation forms to family and patients as needed. Maintain current patient records according to state and company policy. Attend and participate in assigned staff meetings. Prepare verbal presentation for Monday morning staffing describing clinical impression of each patient and family, and present at staffing. Familiarize yourself with and follow all protocol for AMA patients. Familiarize yourself with and follow all protocol for patients who are at risk either of harming themselves or others or are psychologically unstable. Complete other duties as assigned, including but not limited to: substitute for other staff when assigned; lecture to patients and families; monitor lectures and roll call; supervise and coordinate early morning patient programming; train new staff; conduct quality improvement studies; assist in coordinating Serenity Lane special events. Collect UA's as scheduled. Report Blue Flag monitored patients following group session.

Maintain contact with Employers Services regarding Blue Flag patients. Provide referrals when needed. Make phone contact with patients who are not attending R.S. following Their transition from Outpatient. Maintain group sign-in sheets.

QUALIFICATIONS: Minimum 2 years experience. Current CADC
